Countryside Memory Center provides free screening for individuals with memory concerns.

This screening process evaluates your cognitive function and also assesses possible reasons for memory loss. This might include sleep patterns, medication interactions, depression and much more.

All information gathered is sent to our consulting physician who prepares a report for your local provider.

A physician’s referral is not needed to schedule a screening.
